Welcome to the Opella HCP Sampling Portal web site through which U.S.healthcare professionals may request samples from Chattem, Inc. d/b/a Opella.

Prior to using this site, please take time to review the terms below, you are asked to check the "Agree to the Terms and Conditions Above" button below to confirm your understanding. These terms together with the Privacy Statement are the "Terms" for interactions with this web site. Except as expressly provided in other terms and policies on the web site, these Terms constitute an agreement between you and Opella with respect to your use of this web site. Opella reserves the right to update or change these Terms at any time and for any reason without notice. By accessing and using this web site you agree to be bound by any such revisions and should therefore periodically visit this page of the web site. Electronic Signatures

To use the Opella Sampling Portal, you will need an electronic signature which is secured by information provided by you as part of the registration, including a password and PIN of your choice. Use of your electronic signature certifies that you, a licensed healthcare professional in the identified state, have requested the indicated samples for the medical needs of your appropriate patients.

You are solely responsible for maintaining the confidentiality of your password and PIN and agree not to share or give your password and PIN to anyone else. Termination

Governing Law and Jurisdiction

You agree to indemnify, defend and hold harmless Opella , its shareholders, affiliates, directors, managers, employees or other representatives from and against any claims, actions or demands, liabilities and settlements including, without limitation, reasonable legal and accounting fees, resulting from, or alleged to result from, your violation of these Terms or any activity related to your use of this web site by you or any other person accessing the site using your password.

Modification

Neither the course of conduct between the parties nor trade practice shall act to modify any provision of these Terms.

Assignment

Opella US may assign its rights and duties under this Agreement to any party at any time without notice to you and/or your approval.

ORDERING TERMS AND CONDITIONS:

I acknowledge that this registration creates an electronic signature. You are required to create a password and PIN to order samples through this portal. I understand and agree to receiving system generated, transactional emails related to my activity directly on this site. My signature and PIN certifies that I am a health care professional and have requested the above identified samples. Furthermore, I have requested these samples for the medical needs of my patients and I acknowledge that they are not for sale, resale, trade, barter, to be returned for credit or for third party reimbursement. I understand that the information, which may include item value, may be retained by Opella US and submitted to federal/state government as required by law.
